Declaration by Father/Mother/Guardian

1. I hereby declare that the information furnished in this application is complete and accurate to the best of my knowledge and belief.

2. I clearly understand that my child is liable to dismissal for misconduct, habitual idleness, non-payment of fees when due or for any other reason which in the opinion of the school authorities render my child’s continuance in school undesirable.

3. I declare that I will not ask for FEE REFUND under any circumstance, Also, I will pay the fee on time and will not default on installment payment.

4. I solemnly declare that | am aware of the rules and regulations of this school, and will pursue my child to adhere to the same in case of any lapse from my part of my ward to obey the rules and regulations of this school. | agree to any disciplinary action taken against my child by the school authorities.
